I understand that that my registration fee and payments towards my vacation is non-refundable.

I understand that if I do not make monthly payments my package is subject to cancellation.

I understand that I must notify OE of any name changes by June 1 or a $15.00 name change fee will apply.

I understand the event choices made on this form and their price and understand that I cannot drop any choices made.

I do understand that the Final Payment Date is June 20, 2009 (no grace period or postmarks). If I miss this date, I do understand that I will not receive any refund on any monies paid in. If OE allows a grace period, I understand it is a courtesy and if I miss the grace period, I will be canceled without refund on any monies paid in.
